Educational Therapists

1. I/We understand that Occupational Therapy is a health profession within the field of rehabilitation medicine. The profession's focus is on the promotion, restoration and maintenance of productivity in people with a wide range of abilities and disabilities.

2. The therapist(s) are Occupational Therapist trained to work with a client to equip the client with the foundations to be successful in living skills. The consultation, assessment and intervention is directed to enhance the client's ability to be an independent and productive individual in the areas of self care, social, play and school.

3. I/We understand that the therapist will maintain confidentiality of treatment at all times. However, where expertise from another therapist is required, the therapist may share necessary information.

4. At times, treatment involves challenging physical activity; that is, physical activity, which exceeds activity typically, occurs during regular daily living for the client.

5. I/We understand that if I do not completely understand the process of the treatment, it is my responsibility to obtain a clearer understanding of the therapist's objectives and outcomes, and how he/she is trying to achieve them.

6. I/We knowledge that

● there are inherent physical risks involved

● despite reasonable precautions, accidents can occur and the client could possibly sustain personal or physical injury through his/her participation.

7. I/We agree KIDZ Pediatric Occupational Therapy Consultants, its employees, or agents will not be held liable for any damage or injury.

8. I/We will inform the therapist of all pertinent health concerns and physical conditions regarding the client.

9. I/We understand the challenge of physical activities in the course of treatment and hereby release KIDZ Pediatric Occupational Therapy Consultants, its employees, or agents from any claims by me/us.

10. I/We consent to the use of Closed Circuit Television (CCTV) surveillance system to monitor and record the activities of my/our child. I/We understand that the images will be viewed in the waiting area unless I/We inform not to do so.

Please provide an emergency phone contact as well as an alternate.

Having read the above I/we hereby consent to treatment/services that is deemed necessary by the therapist. I understand that it is KIDZ Pediatric's sincere intent to educate me on every process during the treatment program with eventual discharge from their services. I also realize that no guarantees have been made to me as to the result of these services at KIDZ Pediatric Occupational Therapy Consultants Pte Ltd.
